Forms & Policies

As a new patient, you can make your first visit with us go even more smoothly & quick by filling out the initial paperwork ahead of time. Simply print and complete each of the forms and bring with them to your first appointment.

We will work with your insurance and file with the company as a courtesy. The patient should still follow through and check with the company as well.

We will take medical history records as a precaution and make a note of any known allergies or complication in the past.

We will have you sign a HIPPA agreement to ensure the privacy of your information. Please bring a completed form into our office prior to the appointment.

The payment policy outlines the payment methods accepted, allows us to offer financing where available and set up a payment plan if necessary outside of the applied insurance coverage. We also have a cancellation fee that should be reviewed prior to scheduling an appointment.

Should you schedule an appointment and not be able to make it, we do request more than 24 hours notice. A fee will be charged for consistent cancellations without proper notification.
